Diagnostic Letter

A diagnostic letter is a concise alternative to a comprehensive psychological report. It typically includes the reason for referral, relevant background information (including prior diagnoses), a brief outline of assessment tools used, and a summary of key findings. Diagnoses are clearly stated, and recommendations are brief and directly related to the identified condition(s).

These may include suggestions for therapy, support services, or third-party interventions. Diagnostic letters are often used when a focused, formal statement of diagnosis is required—for example, for medical, educational, or funding purposes.
